The Integrated Behavioral Health Therapist is responsible for independently providing assessments, treatment/care planning and psychosocial therapy within a primary care clinic setting for a diverse population of preventative, acute care and chronic disease management patients. The incumbent uses a variety of theoretical treatment models, evidence based practices, skill building and interventions. The individual must use excellent communication skills to work within a primary care/chronic disease management team setting with limited supervision; documents treatment/communication in keeping with department standards and keeps current on community resources to effectively provide linkage and advocacy to fulfill clients’ needs.

Typical office environment with periods of time spent in patient care areas.
Master’s degree from an accredited institution in Social Work, Counseling, Marriage & Family Therapy, or related field. 2 years of experience providing direct therapy is preferred.
Current licensure as a Licensed Clinical Social Worker (PCMSW, CMSW), Licensed Mental Health Practitioner (PLMPH, LMHP, LIMHP). Provisional status may be considered.
Must be able to read, write and follow written and oral instructions and possess positive skills in communication. Ability to deal tactfully with personnel, patients, visitors, and the general public.
Ability to calculate at the level necessary to perform essential duties of the position.
Ability to make independent decisions when circumstances warrant such action. Ability to interpret a variety of instructions furnished in oral, written, diagram or schedule form.
